Wow this is like the bible of diving. I think you made up some hard work here writing up all those things.
But my opinion as Instructor is that it is too much you wrote because it's my job to teach my students all this you wrote, therefore I get paid.

I once had a guy from China who wanted to do a DSD with me he was really scared of everything and he had a 1000 words in his head of people who never been in the water bud gave him advice of how to dive and what to look out for. I he could not do what I say and once I explained him on dry land how to clear a Mask if water comes in, he asked me if we are going to do this underwater, well where else from......
I like customers who come with an open mind to me who can listen to what I have to tell them and do what I tell them. This way I can insure that they are safe and have fun.

I would recommend everyone to read a bit more and listen to the instructor and maybe some of your buddy's but don't take everything like in pre-school. You as a diver are responsible for your actions therefore you can read your dive manual(s), ask you instructor or continue you education, but the one thing nobody can do for you is using your brain. You have to open your mouth if you are unhappy with someones dive service, that way if the Instructor know about they can do something about. People usually call them self instructor because they have a basic idea of what to do (regardless of certificating agency), yes there are a few exceptions but those are not the everyday norm.
The main thing is as "udive" wrote in another post that you as paying customer are happy about the approach the staff of the shop you are intend to dive with makes.

One more thing is about Dive insurances. There should be nothing close to 500 or more $ US
I have a professional liability insurance which covers myself in case of an accident and also covers costs in case somebody sues me. This one cost me about 120EURO (140$US/year) A normal insurance for any diver covering his/her diving in water diving activities should never cost more than 50$ us a year!!!
Check out DAN as the biggest dive insurance companies in the world there are many others doing the same services for maybe even better prices.
You for sure missed out on diving in Okinawa, Japan....
